TORDIA - RCT para depresion adolescente resistente a SSRI (N=334). Diseno factorial 2x2: switch a SSRI alternativo vs venlafaxina, con o sin CBT adyuvante. Anadir CBT al cambio farmacologico aumenta respuesta de 40.5% a 54.8% (NNT=7). El cambio entre SSRI vs venlafaxina no difiere en eficacia pero venlafaxina tiene mas efectos adversos. Define el algoritmo de manejo de depresion adolescente refractaria.
